Turn the key enough to start the fuel pump, give the gas pedal a couple of
pushes all the way to the floor, then engage it the rest of the way with the
pedal half way down.  Sometimes I have to move the gas pedal a little
between nothing and 1/4 - 1/2 while it's cranking.
