 this is how I got mine out:
   drill out the rubber bushing (you can also burn out the rubber with a torch,
     but I didn't want to stink things up that much).
   take a hacksaw apart, thread the blade through the eye, and reassemble.
   cut through the sleeve with the hacksaw (this takes a while because there
     isn't room to take full strokes with the saw).
   use a hammer and chisel to drive the sleeve out
